Dr Alain Vuylsteke BSc, MA, MD, FRCA, FICM is a Consultant in Intensive Care and Clinical Director at Papworth Hospital, Cambridgeshire. Daniel Brodie, MD is Associate Professor of Medicine at Columbia College of Physicians and Surgeons, NewYork-Presbyterian Medical Centre, New York.
