Periodontitis is an infectious disease characterized by inflammatory changes in the tissues surrounding to teeth leading to periodontal attachment loss and alveolar bone destruction. The aim of the periodontal therapy is total treatment, that is, the coordination of all treatment procedures for the purpose of creating a well-functioning dentition in a healthy periodontal environment. Successful treatment of periodontitis is associated with a substantial change in the proportional composition of the subgingival microbiota, usually correlated with improved clinical conditions. Scaling and root planing has been shown to be an effective treatment for chronic periodontitis. Although mechanical treatment significantly decreases the prevalence and levels of subgingival microorganisms, it does not necessarily eliminate all pathogens.Periodontal pocket provides a natural reservoir bathed by gingival crevicular fluid that is accessible for the insertion of the delivery device. Local delivery systems, when retained in the periodontal pocket, can release the antimicrobial agents at levels that can be delivered by systemic antibiotics with minimal side effects.
